It was finding out that the much-hyped medications didn’t really work for her. “I have been on Wegovy for a year and a half and have only lost 13 pounds,” said Griffin, who watches her diet, drinks plenty of water and exercises regularly. “I’ve done everything right with no success. It’s discouraging.” In clinical trials, most participants taking Wegovy or Mounjaro to treat obesity lost an average of 15% to 22% of their body weight — up to 50 pounds or more in many cases. But roughly 10% to 15% of patients in those trials were “nonresponders” who lost less than 5% of their body weight. Now that millions of people have used the drugs, several obesity experts told The Associated Press that perhaps 20% of patients — as many as 1 in 5 — may not respond well to the medications. It's a little-known consequence of the obesity drug boom, according to doctors who caution eager patients not to expect one-size-fits-all results. “It's all about explaining that different people have different responses,” said Dr. Fatima Cody Stanford, an obesity expert at Massachusetts General Hospital The drugs are known as GLP-1 receptor agonists because they mimic a hormone in the body known as glucagon-like peptide 1. The drug helped reduce her appetite and lowered her risk of diabetes, but she saw little change in weight. “It’s an emotional roller coaster,” she said. “You want it to work like it does for everybody else.” The medications are typically prescribed along with eating behavior and lifestyle changes. It’s usually clear within weeks whether someone will respond to the drugs, said Dr. Jody Dushay, an endocrine specialist at Beth Israel Deaconess Medical Center. Weight loss typically begins right away and continues as the dosage increases. For some patients, that just doesn't happen. For others, side effects such as nausea, vomiting and diarrhea force them to halt the medications, Dushay said. In such situations, patients who were counting on the new drugs to pare pounds may think they’re out of options. “I tell them: It's not game over,” Dushay said. Trying a different version of the new class of drugs may help. Griffin, who didn't respond well to Wegovy, has started using Zepbound, which targets an additional hormone pathway in the body. After three months of using the drug, she has lost 7 pounds. “I'm hoping it's slow and steady,” she said. Other people respond well to older drugs, the experts said. Changing diet, exercise, sleep and stress habits can also have profound effects. They will be planting hatchery trout from now through spring in many local lakes, and the season kicks off with the Black Friday trout opener. For over a decade the popular program has aimed to get families and friends out together for a day of fighting fish, instead of fighting the holiday shopping crowds. State-wide there will be 24 lakes stocked with “jumbo” rainbow trout before Nov. 29, ranging up to 17 inches in length, with some of tipping the scales at three pounds. 65,000 trout will be stocked out, mostly in the western part of the state. Stacie Kelsey of the WDFW Inland Fishes Program reports that the same lakes will soon be stocked in Region 5 as in recent years. Each of them will receive 2,000 of the jumbo-sized trout. “Those are the best lakes for the area, and they seem to produce well,” Kelsey said. She said the program seems to get more popular every year. “We do get a lot of good feedback about it,” she added. Two lakes in Clark County will be stocked for the event: Klineline Pond, in Vancouver’s Salmon Creek Park, and Battle Ground Lake, in Battle Ground Lake State Park. Kress Lake in Cowlitz County will also receive 2,000 jumbo rainbow trout. Two Lewis County lakes will also be stocked, the Fort Borst Park Pond, and the South Lewis County Park Pond. In Klickitat County Rowland Lake will be stocked. In addition to the Black Friday stockings, many lowland trout lakes will also be planted with “catchable” rainbow trout. These fish run from 10 to 12 inches. In the months to come, plenty of trophy sized brood trout will also be stocked out, providing anglers a shot at trout from five to 10 pounds. In Clark County, Klineline Pond has already been stocked with 2,000 catchable trout. Kelsey reports that in both Battle Ground Lake, and Klineline Pond, the trout seem to prefer bait to artificial lures. And, while Berkley Power Bait has always been the most popular offering, the trout already stocked in Klineline Pond are showing a preference for a different bait right now. “For some reason the bite on Power Bait has been kind of slacking off there,” she said. “Now it’s the Berkley Trout Nuggets that are catching fish.” She added that some anglers have been fishing with a combination of a worm tipped with a salmon egg. Klineline Pond has excellent bank access, while Battle Ground is a good lake for small boats, and there is a fishing pier there as well. The two lakes in Lewis County that get the Black Friday stockings tend to fish very well, too, according to Kelsey. “Fort Borst Park is very popular with families and kids,” she said. “It’s got pretty good shore access all the way around it.” The South Lewis County Park Pond also has very good shore access, and there is a boat launch and a fishing pier. In the Columbia River Gorge, Rowland Lake is very popular on Black Friday, and it is a larger lake with a boat launch and good bank access. Lures and bait both work well here, and Kelsey reports that these fish are very aggressive. “Rowland is really popular, and every year it’s just such a good lake for the Black Friday fishery,” she said. The trout at Rowland seem to take just about every kind of offering, including trolling small plugs in 2 or 2.5 inches, casting or trolling spinners and spoons, or fishing bait below a bobber, or on the bottom. A few other local lakes have already been stocked with catchable trout, including Lacamas Lake near Camas, which has already received 4,000 catchable rainbow trout. Also, Icehouse Lake near the Washington end of The Bridge of the Gods near Stevenson received 2,000 catchable rainbow trout on Nov. 13. Icehouse Lake has been fishing very well with spinners recently, but over time the trout will switch to preferring bait. Kelsey also mentioned that just ahead of Christmas, Sacajawea Lake in Longview will once again receive a strong stocking of jumbo trout that will rival the big rainbows stocked out for Black Friday. This fishery has also become very popular in recent years. The enthusiasm around the Black Friday stocking program is another reason that trout fishing is the most popular kind of fishing in Washington state. “I think it’s going to be another great year,” said Kelsey, “and we just encourage people to get out and have fun with their families. It’s a great time to take a kid fishing, or a parent fishing.” Buzz Ramsey’s winter trout tips Few people get more excited about this fishery than local fishing legend Buzz Ramsey, who was one of the original promoters of the program. Listed here are some of his tips for getting the most out of this event. 1. Sleep in and enjoy the late-day bite. “A guy can start early, but it seems like the best bite starts about 9 or 10 o’clock,” Ramsey said. “The water is cold, just a slight rise in temperatures as the day progresses, even a fraction of a degree, seems to turn those fish on. I’ve seen it time and time again.” He reports that the best fishing seems to be mid-day. 2. Smaller baits work better in the cold water. “The fish are not real aggressive in the cold water,” he said. “Powerbait is the overall favorite, but the trout will take a smaller bait better. For the best success, try using a small enough hook and just big enough bait that it floats off the bottom, so the cruising trout can find it. Too big a hook and it will sink to the bottom. It can get bit that way, but it’s not as effective.” He suggests a size 18 treble hook, and just enough bait to float it off the bottom. Using a free-sliding sinker and some slack in the line will also help. The fish can grab the bait and swim and off taking the bait deeper while not feeling that weight on the line. “Set the hook,” said Ramsey. “Don’t forget that.” 3. Troll with small plugs, spinners, or spoons. “I have caught them on everything, a 2.5 flatfish, a little Maglip, or the Spinfish in the 2.0 or 2.5 size,” he said. “I stuff the Spinfish with tuna and troll them behind a small lake troll flasher. I have also caught them trolling Rooster Tails, and the Thomas Minnow. He reminds anglers that using some scent on your bait can increase the bites, and tipping your spinner with a small bit of Berkley Trout Worm, or some other attractant can increase your success. 4. When it is warm and windy, fish the windward shore. “If you get some afternoon sun, those fish will come into the shallows, not real shallow, but they will move into that warm water,” Ramsey said. “Also, if you get a west wind on a sunny day, it blows that warm water onto the east end of the lake. It can be good to fish the windy end of the lake when that happens. As 2024 concludes, 24 of the 27 rest stops have reopened since construction began in 2021. Most of the service areas were demolished and rebuilt, while four had significant renovations. Applegreen, which was awarded a 33-year contract to operate the rest stops, is leading the construction project. The changes to the service areas include a new restaurant lineup — Chick-fil-A, Popeyes and Shake Shack are among the fast-food options at the rest stops — and other amenities, such as dog walking areas and electric vehicle chargers. The $450 million project is funded by private sources. No tax or toll dollars have been used to finance the work. The project got off to a slow start in 2021, with delays caused by COVID-19 and supply chain issues . The first rest stops that closed for construction reopened in the third quarter of 2022, more than a year after the project began. As the pace of the projects improved, more rest stops reopened this year. In January, about half of the rest stops (13) had been rebuilt and were open to the public. By the end of the year, 11 more reopened. The final three rest stops are scheduled to reopen at different points in 2025. Angola Service Area, one of the largest Thruway rest stops, is slated to reopen in the first quarter. Angola is the westernmost service area and accessible from both sides of Interstate 90. Mohawk Service Area will also reopen in the first quarter, according to the Thruway Authority. Modena Service Area is along Interstate 87 and will reopen sometime in the fourth quarter of 2025. The story so far: Britain’s House of Commons on November 29 voted by a majority to legalise assisted dying — a less controversial synonym for assisted suicide in England and Wales. The step signals a seismic social shift on an emotive issue. Advocates regard the new Bill as a humane and compassionate intervention that should — for those who want to exercise the option — bring closure to a painful, debilitating and degrading dependence on the immediate family. Opponents, who include the head of the Church of England, are apprehensive that the severely disabled and vulnerable patients may feel forced to end their lives to relieve the burden on relatives. “It should never become easier to seek help to die than to live,” they say. The debate on the Bill’s merits has also brought to the fore concerns that the delivery of hospice and palliative care services could be undermined. Portrayed sometimes as a “Give us our last rights” campaign, about 75% of the public has backed the liberalisation of assisted dying, as per a U.K. National Centre for Social Research survey. How controversial is the move? The U.K. courts have time and again maintained that given the sensitive ethical, philosophical, and social questions involved, the subject of assisted dying was not a matter on which the judiciary could pronounce a decision based on concrete evidence. Successive British governments have similarly maintained that any change to existing law was for parliament to consider and a matter of their conscience for individual MPs and that the government would officially adopt a neutral stance. The Terminally Ill Adults (end of life) legislation was thus brought forward as a private member Bill by the Labour MP Kim Leadbeater. Among its supporters were Prime Minister Keir Starmer, the Home Secretary, the chancellor of the exchequer, and the Opposition leader Rishi Sunak. Six Cabinet Ministers opposed the proposal. What is the current law in this area? Suicide, (self-inflicted death), or attempted suicide are not in themselves criminal offences in England and Wales. Conversely, an act that encourages or assists a person to commit suicide or attempted suicide is a criminal offence that carries a 14-year prison sentence under the 1961 Assistive Suicide Act. What is the proposed law? The Bill before parliament seeks to give terminally ill, mentally competent adults who have less than six months to live — which assumes that a reliable method exists of forecasting survival — the option to end their life. Under one stipulation, any such request must be authorised by two doctors and a High Court judge. What were the petitioners’ prayers? Since 2001, severely disabled applicants have petitioned the U.K. courts that the 1961 law, as well as the prosecution’s refusal to allow them to end their lives under medical assistance, was incompatible with their rights under the European Convention on Human Rights (ECHR). At issue was the right to life (Article 2), which the petitioners viewed as entailing the right to self-determination and an entitlement to end life with assistance. They have also argued that the denial of the option to die with assistance — insofar as it failed to alleviate suffering — amounted to cruel and degrading treatment prescribed under Article 3. It moreover constituted an infringement of the right to privacy and freedom of conscience enshrined in Articles 8 and 9. Finally, as persons with disabilities who needed assistance to end their lives, the denial of permission by the prosecution amounted to discrimination, in breach of Article 14 of the ECHR. What did the court rule? The U.K. House of Lords in 2002 unanimously overruled these contentions. But, in a 2014 Supreme Court decision, the majority opined that although the 1961 law was incompatible with the rights of two incurably suffering but not terminally ill petitioners, it was for parliament to modify the law. In 2015, the European Court of Human Rights ruled that the U.K. Supreme Court was under no obligation to examine the merits of a challenge to primary legislation and that Britain’s government was open to concluding that this was a matter for parliament to arrive at a suitable determination. It is thus crystal clear that only new legislation can alter the status quo in this matter. What is the status outside? The crown dependencies of Jersey and the Isle of Man are close to enacting legislation to allow assisted dying for the terminally ill in the next two years. Scotland, where a separate bill is under the active consideration of political parties, is deliberating whether the subject matter falls within the realm of competence of Holyrood, the Scottish devolved parliament. Following Switzerland, the first country which in 1942 legalised assisted dying, unless driven by selfish motives, societies seem to see wisdom on compassionate grounds to allow people to end their lives. The 1997 Death with Dignity Act in the state of Oregon, which allows mentally sound adults with terminal illness and six months to live, to end their lives through self-administered drugs, has broadly set the tone for legislation within the U.S., most of Australia and New Zealand. Canada’s 2016 Medical Assistance in Dying law may be availed even by those whose condition is not terminal, if it is grievous or irremediable. The relevant 2002 law in the Netherlands grants immunity from punishment for doctors for euthanasia under strict conditions. Belgium’s 2002 act also includes psychiatric illnesses and since 2014 has been extended to minors with parental consent. Spain and Germany have enacted laws but are at various stages of enforcement. Legislation is still pending in France and Ireland. What is the position in India? In Common Cause vs. Union of India (2018), the Supreme Court affirmed that the “right to die with dignity” is integral to the right to life and personal liberty under Article 21 of the Constitution. The court also legalised “passive euthanasia,” which involves the withdrawal of life support from terminally ill patients or those in a permanent vegetative state. This allows individuals to die a natural death in the absence of medical interventions. The top court also issued comprehensive guidelines to facilitate passive euthanasia. For instance, if a patient leaves behind a living will, it must be executed in the presence of two witnesses and attested by a Judicial Magistrate. In 2019, the Indian Society of Critical Care Medicine filed an application seeking modification of these guidelines, arguing that they were onerous and impractical. Accordingly, a five-judge Constitution Bench in 2023 simplified the process by imposing strict timelines at each stage and limiting the role of the Judicial Magistrate. In September, the Health Ministry released draft guidelines on withdrawing medical support for terminally ill patients, addressing a regulatory gap that had left healthcare professionals in the lurch. These specify that doctors must refrain from initiating life-support measures when they offer no benefit to the patient and are likely to cause suffering and a loss of dignity. DA wants to remove brand label of imported riceHyderabad: In spite of the Telangana govt expected to fall short of its targeted procurement of fine variety paddy (fine rice) from the kharif crop, the state is not allowing fine rice to be brought in from neighbouring states, including Andhra Pradesh, to procurement centres. Recently, as many as 16 lorries of fine variety paddy were stopped in a night in Khammam on the Telangana-AP border. Traders from Andhra Pradesh and Karnataka are trying to sneak in lorries with fine variety paddy into Telangana as the govt here is offering a 500 bonus per quintal of fine variety paddy, in addition to the minimum support price. The Congress govt needs 36 lakh metric tonnes (LMT) of the fine variety paddy for supply through ration shops. After milling, the state gets 24 lakh metric tonnes of rice. The 24 LMT fine rice is the annual requirement of Telangana to supply rice through ration shops to over 89 lakh ration card holders. If the state falls short of the targeted 36 LMT of fine variety paddy procurement from kharif, it would have to make alternate arrangements to fill up the gap. Till date, the govt has procured 18.07 lakh metric tonnes of fine variety paddy from farmers, and major arrivals of paddy are likely to come to an end by Dec 31. Over 300 check posts armed with CCTV cameras have been set up on the Telangana-AP, Telangana-Karnataka borders and other states. Traders have expressed their ire over the state govt not allowing their paddy-laden trucks to enter Telangana. "As per rules, rice millers have to mandatorily mill 50% of the govt-given paddy, and the remaining 50% they can purchase from the market and mill it," a trader said. However, officials said the main reason for traders flocking to Telangana with paddy stocks is due to the 500 bonus. The name of the farmer, details of the location where paddy was produced, and generation of OTP on the farmer's mobile number are some of the measures being implemented by the civil supplies corporation to check paddy from other states being sold at procurement centres. Civil supplies commissioner DS Chauhan said the 500 bonus was meant only for Telangana farmers and that they would manage fine rice supplies to ration shops as ‘sanna biyyam' (fine rice) in huge quantities was expected to be procured from the ensuing rabi crop too.
