The final report of the Commission to Study Telehealth Services, a group that was established by the Legislature in 2020, says it should be legal for telehealth providers to prescribe certain medications for mental health and substance use disorders without requiring an in-person visit. “Those types of conditions are treatable through this modality. You don’t need to necessarily lay hands on the patient in order to treat them. It fits very well,” said Dr. Jonathan Ballard, chief medical officer for the state Department of Health and Human Services. In medicine, “modality” means “method of treatment.” Data from 2023, for example, showed that between one-third and one-half of doctors’ visits for mental health and substance use disorders in the state were done via telehealth, compared to 5 percent or less for other types of visits. With that in mind, the commission’s most immediate recommendation is that legislators expand a provision created by Gov. Chris Sununu’s emergency action during the COVID pandemic and later codified into law, which allows providers to prescribe certain drugs for opioid-use disorder without an in-person visit. Similar online-only prescribing should be allowed for a variety of mental health and substance-use disorders, the commission said. The group’s final report was released to Gov. Sununu and legislators on Nov. 26. Ballard said the recommendation was spurred by data and experience from Dr. Audrey Kern, an addiction medicine specialist who was a member of the commission. “For example, a person with anxiety may be treated with controlled medication. They’re perfectly stable but still have to come in for an office visit. Attention deficit disorder is another (example),” Ballard said. The commission found that the use of telehealth services has declined slightly since the pandemic lockdown, when the practice soared from almost zero. It now sits at roughly 60-70 visits per 1,000 members, with Medicaid patients slightly more likely to use online services than private insurance patients. “We were reassured that there was still continued use (of telehealth), settling into an amount that is clinically appropriate, driven by need,” Ballard said. He pointed to the fact that there’s a winter uptick, when it gets harder to drive to the hospital, clinic or doctor’s office. “There’s a spike every single winter. That reassured us the data is valid,” he said. Telehealth is most often used in the populated southeast part of the state, which isn’t a surprise even though it may be of more value in rural areas with less access to providers. “So much of this data follows the map of other health service utilization. You have more providers in the southern part of the state which drives use in all health services, including telehealth,” Ballard said. The commission also recommends that lawmakers extend the group for two more years because many new technologies are coming online that can help telehealth services. Ballard pointed to a movement known as Hospital-at-Home being tested in Massachusetts and other places. “You can treat people with a hospital level of care using advanced technology” such as telemetry of a patient’s vital signs at home that can have a nurse or doctor called there if necessary. “It can let you move people out of the hospital or completely avoid a hospitalization,” he said. “It’s very promising.”

The person spoke to The Associated Press on condition of anonymity Tuesday night because the deal is subject to a successful physical. The two-time Cy Young Award winner broke the news personally by posting a photo of himself on social media in a Dodgers uniform — No. 7. Snell gets a $52 million signing bonus, payable on Jan. 20, and annual salaries of $26 million, of which $13 million each year will be deferred. Because Snell is a Washington state resident, the signing bonus will not be subject to California income tax. Snell would join two-way star Shohei Ohtani and fellow Japanese right-hander Yoshinobu Yamamoto atop Los Angeles' rotation, giving the Dodgers the first megadeal this offseason following Ohtani's $700 million, 10-year contract and Yamamoto's $325 million, 12-year agreement last offseason. Ohtani didn't pitch this year while recovering from right elbow surgery but is expected back on the mound in 2025. Because he turned down a qualifying offer from San Diego last November, the Giants were not eligible to give Snell another one and won’t receive draft-pick compensation. The Chargers quarterback did not practice Wednesday as Los Angeles began preparations for its game Sunday against Tampa Bay. Herbert injured his left ankle during the first quarter of last Sunday’s 19-17 loss at Kansas City. Herbert said Wednesday that the injury occurred during a 7-yard scramble on third down during the opening drive. Television cameras showed him grimacing and walking slowly to the sideline after the play. “It was difficult to play with,” he said. “It was one of those things where we limited some of the runs out of the pocket. I didn’t feel great, but it was one of those things to play through.” Herbert’s left leg was later bruised after taking a hard hit from linebacker Nick Bolton during the second quarter. Herbert missed only one play and completed 21 of 30 passes for 213 yards and a touchdown. “The contusion, I think that is something that is easily recoverable. I’m doing everything I can with the ankle,” Herbert said. “If I felt like I could have practiced at 100% and make sure everyone was able to get full-speed reps, I would have. In a six-page court filing in a Washington federal court, Smith’s team argued that the Department of Justice has long argued “that the Constitution requires that this case be dismissed before the defendant is inaugurated,” referring to Trump’s recent election victory. “This outcome is not based on the merits or strength of the case against the defendant,” the filing states. His office said that prosecutors have conferred with Trump’s attorneys, who indicated they do not oppose the government’s motion. “Based on the department’s interpretation of the Constitution, the government moves for dismissal without prejudice of the superseding indictment,” the court documents state. At the same time, in an appeals court, Smith also wrote he is dropping his appeal of U.S. District Judge Aileen Cannon’s July decision to dismiss a case that accused the president-elect of illegally retaining classified records and allegedly obstructing an investigation. “Dismissing the appeal as to defendant Trump will leave in place the district court’s order dismissing the indictment without prejudice as to him,” his filing said. But his appeal concerning two other defendants in the case, Walt Nauta and Carlos de Oliveira, “will continue because, unlike defendant Trump, no principle of temporary immunity applies to them.” Cannon had dismissed the case after agreeing with arguments that Smith was not lawfully appointed as special counsel. Smith in August asked the U.S. Court of Appeals for the 11th Circuit to reverse her decision. The move marks an end to Smith’s criminal pursuit of Trump over the past two years or so accusing him of attempting to illegally overturn the 2020 election. Smith accused Trump of mishandling classified documents in a separate case, which was dismissed over the summer by a federal judge. The decision was anticipated after Smith’s team said in court filings that it was assessing how to wind down both the 2020 election interference case and the classified documents case in the wake of Trump’s win on Nov. 5 over Vice President Kamala Harris. According to Smith’s team, the DOJ believes that the president-elect can no longer be tried in accordance with longstanding policy that says sitting presidents cannot be prosecuted. The DOJ policy that Smith is referring to, dating back to the 1970s, holds that a criminal prosecution of a sitting president would violate the U.S. Constitution by undermining the ability of the country’s chief executive to function. U.S. District Judge Tanya Chutkan, who is overseeing the case, will still have to approve the request from prosecutors. Trump has often criticized the two cases that were brought by Smith, named as the special counsel by outgoing Attorney General Merrick Garland, describing them as attempts to use the DOJ to target a political opponent. Earlier this year, Trump told a radio host that if he were elected, he would move to remove Smith as special counsel. Smith’s election case ultimately resulted in a July 1 U.S. Supreme Court decision that said presidents can enjoy some immunity from prosecution for their official acts and duties. Months later, Smith filed a superseding indictment that argued that Trump acted on his own accord and not within his presidential duties when he allegedly broke the law. Trump had pleaded not guilty to those charges. Trump’s lawyers had previously said they would seek to dismiss the charges based on a Supreme Court ruling in July that former presidents have broad immunity from prosecution over official actions taken while in the White House. Trump also faced similar, election-related charges in Fulton County, Georgia. However, that case is in limbo after a co-defendant accused the prosecutor, Fani Willis, of being in a romantic relationship with her special counsel, Nathan Wade, who resigned earlier this year after a judge issued an order. That Fulton County judge allowed Willis to remain on the case. Trump and several co-defendants petitioned the Georgia Court of Appeals to reject the judge’s ruling, effectively pausing the case. In Trump’s business records case in New York, sentencing was postponed indefinitely last week by a judge after Trump’s election win. On May 30, Trump was convicted on 34 counts of falsifying business records in connection to payments he made during the 2016 presidential campaign, which he had denied were illegal. Sentencing in that case was initially scheduled for July but was postponed until Nov. 26. When VHS tapes hit the scene in the ‘80s, people thought cinemas would die a natural death in a matter of a decade. But what happened was just the opposite; VHS faced a natural death and CDs took over by the 2000s to be quickly replaced by the DVDs and Blu-ray. Then the internet gave us Netflix and the rest is history. Forty years later, theatres are still relevant. IMAX and 3D have broadened cinematic possibilities thanks in part to innovative technology, big studio budgets and all-star casts. But this is not about commercialised cinema. Instead, we went in search of two cinema halls well past their glory days that have survived (in some form) to the present day. Although the stories are different, they lend a unique perspective to Sri Lanka’s shifting socio-political narrative down the years. Welcome to the Ritz and the Rio – a tale of two cinemas. Ritz cinema transformed The Ritz is in the heart of Borella. Covered in billboards, the theatre stays relevant showing Sinhala and Kollywood releases. The Gallery downstairs has been converted to a pub, while the balcony has been converted into one entire floor for screenings. The lobby of the Ritz cinema is decorated with posters of silver screen classics like Sweep Ticket and Maruwa Samaga Wasey. Between the theatre’s ‘vomitories’ stood a defunct 35mm projector; a reminder of the cumbersome analogue era. Rakith Sugathadasa is the current proprietor of the Ritz cinema which has been in his family for three generations. As a millennial, he is well aware of the challenges to small independent theatres. One issue, he says, is parking. Decades ago, not many people owned cars and so building parking lots were not given much attention when designing commercial buildings. Rakith’s grandfather opened the Ritz in 1959 but passed away two years later and the cinema was rented out till 1980 after which Rakith’s father managed it until 2007. In its heyday, the Ritz was a massive theatre with a seating capacity of 700. “After the popularity of multiplexes, I decided to make it into a 116-seater. That’s why the Ritz survived and the Lido cinema didn’t,” Rakith said. One of the main Sinhala movie centres since the ‘80s, the Ritz cinema is an independent cinema and is part of the Lanka Film Distributors (LFD). Only five film circuits have the authority to release local and foreign films in Sri Lanka – They are the MPI, EAP, CEL, LFD which are owned by the private sector, and the Rithma Film Circuit owned by the Government under the National Film Corporation. In a world of multiplexes the Ritz keeps running due to its budget rates, according to Rakith. “We also have a bar and restaurant downstairs, so that is also one reason we are still surviving”. The Ritz upgraded to digital in 2016 to keep up with the times because analogue is just not feasible to operate. “35mm film costs Rs. 300,000 per single cinema, but a circuit can distribute to every cinema digitally for under Rs. 500,000. Moreover, film rolls are a hassle to operate”. Rakith said he hopes to upgrade the theatre and is still managing the Ritz cinema since it’s a family heirloom. “I grew up here. The Ritz is just a hobby. I have other businesses,” he said with a smile. “At the end of the day, the cinema is not marketable by itself. Films are what drive the market. We have a consistent crowd and sometimes get houseful in the weekends or during holidays”, he added. Rio theatre: flawed beauty In downtown Slave Island stands a decaying building—a flawed beauty and a tragedy of Sri Lanka’s dark past. The Rio theatre and hotel opened its doors in 1965 and featured Sri Lanka’s first 70mm Todd-AO projector. The Todd-AO 70 mm widescreen film format was developed by Mike Todd and the Naify brothers. The Todd AO also marked the beginning of what later became ‘Surround Sound’. The Rio’s first screened the musical South Pacific which was attended by the then Governor General William Goppalawa and a 20-year-old Chandrika Bandaranaike Kumaratunga. During its heyday the Rio screened 70mm hits such as Can Can, Alamo, Sound of Music, Cleopatra and West Side Story. In the 60s, the Rio cinema became a popular entertainment hub for Colombo families as it featured amenities such as restaurants and ice cream shops. “But the good times came to an end quickly when the Film Corporation took over all distribution and importation of films and then the quality of movies began its decline from then,” Rathnarajah ‘Thambi’ Navarathnam, proprietor and manager of the Rio cinema, said. “Hollywood didn’t want to deal with a monopoly; they were big enough to ignore Sri Lanka and the 360 cinemas islandwide dropped to a mere 140 in the following years. However, to save the industry, a few private entities were once again allowed to import films. Torched The Rio was torched during the communal riots of 1983 just as many other Tamil-owned businesses. The cinema was rebuilt but the once-magnificent 70mm Todd-AO was replaced by the basic 35mm projectors, marking the end of an era. Rio earned a bad reputation after its fall from grace. Protections for workers and reducing seabird bycatch are critical measures being sponsored by New Zealand when the 21st Western Central Pacific Fisheries Commission (WCPFC) meeting, which begins Thursday in Suva. The WCPFC brings together Pacific Island states and the distant-water fishing nations to discuss the welfare or otherwise of fish stocks, particularly the various tuna species, in the region. New Zealand intends to lead or co-lead on three issues. In a statement, the Ministry for Primary Industries said New Zealand wants a new measure adopted requiring fishing vessels to implement binding crew labour standards. It said New Zealand is leading work to strengthen the rules to prevent the capture of seabirds by long line vessels fishing in the region. New Zealand is also backing the implementation of standardised electronic monitoring on vessels. It said this would mean data can be collected for compliance monitoring and be used for research. The Ministry said New Zealand also wants to see improvements to the rules governing transhipment at sea, particularly for longline vessels. Non-governmental organisations, such as, the Pew Charitable Trusts are also strongly in favour of these measures, and say clearer rules around transshipment will help.None
